In order to register you need to submit a copy of your Baptism Certificate and be ready to supply other Sacramental Information.

Please have the following information ready when you register:

• Birthplace (City/State)
• Name of the Church where you were Baptized
• The full mailing address for the Church
• Your Baptism date
• Your mother's maiden name
